Pennsylvania Child Abuse History Clearance:
On the left side of
the main page click on “Find a form” , scroll down to “Other Topics”,
Click on “Child Abuse History Clearance Forms”, or
use this link: http://www.dpw.state.pa.us/findaform/childabusehistoryclearanceforms/index.htm
choose the “English Child Abuse Clearance Form” and
complete.
Also, right below you can find the link to the Criminal
Record Check form. And don’t forget about the FBI Criminal
Background Check (Fingerprints)!!!!! On this same page you will
find the link to start this process on-line. So basically, on
this one link, they can find everything they need:
